PCW (Partnership Community Worker)

The PCW (Partnership Community Worker) is part of a diverse health network based in community organisations and high schools. PCW’s work in collaboration with general practice teams and their local communities to assist whānau access primary health care through identifying barriers and planning solutions. PCWs focus either on people who are not enrolled in a general practice or who are enrolled but are not visiting their general practice teams as often as they need.

PCW services include:

  • assistance with health appointments; transport and attendance
  • support clients to address other needs that are impacting on their health, e.g., housing, benefits, social isolation
  • link clients with other community support services
  • provide referrers with information and resources about supports available in the community
  • provide cultural support and interpreter services.
